MUAP (Motor Unit Action Potential)

MUAP analysis의 기본

  (1) Amplitude: 0.2 - 3mV

  (2) Duration: 6-15ms

  (3) Rise time: < 0.5ms

    - initial positive deflection 에서 negative deflection 의 peak 까지 걸리는 시간으로, motor unit 까지의 거리를 말함

  (4) Turn/Phase: phase는 보통 2-3개

   - muscle fiber의 숫자 및 Synchrony 를 반영함 

  (5) Recruitment

   - Minimal contraction 시 4-5Hz single MUP가 발생. 힘을 조금씩 더 주다보면 MUP의 frequency가 올라가기 시작하고,

     새로운 MUP가 동원됨. 이때의 Firing rate를 recruitment frequency라고 하고 보통 8-10Hz 에 해당함

  - Recruitment ratio: Recruitment frequency / MUP 종류 로, 보통 5:1을 보임

  - Maximal firing frequency는 30-50Hz이고, 이는 곧 5-6종류의 MUAP가 보여야 정상이란 뜻.

 

 Recruitment에 대한 평가

  - Reduced / Normal / Early 로 나뉨

  - Reduced recruitment는 neurogenic change 때 나타나는데, 병리기전을 생각해보면 됨.

   힘을 주면 줄수록 frequency가 높아져도, 동원되는 MUP의 종류는 한정되어 있음.

   따라서, recruitment ratio가 높게 계산되고, 이를 두고 reduced recruitment라고 함 (5보다 높음)

 

 * 다만 이때 poor cooperation에 의한 reduced recruitment를 감별해야 함.

 

 - Early recruitment는 myogenic change 때 나타나고,

  마찬가지로 병리기전을 생각해보면, myogenic change 땐 힘을 조금만 줘도 동원되는 MUP 종류가 많고,

  frequency는 높지 않아서, recruitment ratio가 작게 계산됨(5보다 작음).

 

 * Interference pattern이란,

   full contraction 시 나타나는 MUAP의 overlap 패턴으로,

   full / reduced / discrete로 나누어짐.

  Full이 정상이고, discrete는 각각의 MUP를 구별할 수 있는 경우를 말하고, reduced recruitment를 보일 때 이렇게 나타남.

  reduced interference pattern은 full- /discrete interference pattern의 사이 단계라고 보면 됨.
